Form Lighting Warranty
At a glance: 2-year limited warranty on every Form Lighting product against manufacturing defects and functional failure, plus free lifetime tech support
This is Form Lighting's policy, not an NVMOS policy. We've summarized Form Lighting's published warranty in plain English. The manufacturer's official terms always govern and can change without notice. Last reviewed on nvmos.com: October 15, 2026.
Coverage by product
| Product or component | Coverage |
|---|---|
| All Form Lighting headlights, tail lights, fog lights and accessories: manufacturing defects or functional failure | 2 years from purchase |
| Product support and troubleshooting, including after the warranty runs out | Free, for life |
| Moisture beyond normal condensation (running droplets or puddling) | Covered at Form's determination |
| Consumable parts such as incandescent bulbs | Not covered |
What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Projector Headlights — Toyota 4Runner (2014–2024)
The stock 4Runner headlamp is a chrome reflector with a single projector. This replaces it with three: two bi-LED projectors for low and high beam, plus a third that adds light when the high beams are on. The housings are blacked out, the lenses are UV-coated polycarbonate, and the assembly is IP67 rated.
At startup the running lights play a two-stage chasing sequence, then hold in cool white or amber, your call. Turn signals chase from the inside of the lamp outward. Adapters are in the box for 4Runners without factory daytime running lights, and everything plugs into the factory connector.
| Spec | Detail |
|---|---|
| Optics | Three projectors per headlight: two bi-LED (low/high), one high-beam supplement |
| Functions | Low beam, high beam, DRL, sequential turn signal, startup sequence |
| DRL | Selectable cool white or amber |
| Turn signal | Sequential amber, inside to outside, returns to DRL |
| Install | Plug-and-play into the factory connector; 1–2 hours, basic tools, no modification; non-DTRL adapters included |
| Compliance | Meets SAE, DOT, FMVSS and CMVSS standards for on-road use in the US and Canada when installed and aimed properly |
| Assembly | Assembled in an IATF 16949-certified facility |
| Warranty | 2 years, US-based support, replacement parts ship from a US warehouse |
| Fitment | Toyota 4Runner 2014–2024; sold as a left/right pair |
| Part number | FL0017 |
