What's covered

- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Reflector Headlights — Toyota Tundra (2014–2021)
These are full-LED reflector headlights for the 2014–2021 Tundra, styled to match the factory LED lamps Toyota offered from 2018. Getting that look from Toyota parts means re-pinning connectors and adding harnesses; this kit does it with plug-in adapters instead.
LED lines along the top and bottom edges form the running light, and amber LEDs in the same area chase when you signal before returning to the white DRL. The kit includes a DRL harness, halogen adapters, LED adapters and resistors, so it plugs into either factory setup. If you prefer a projector beam, Form also makes a projector version for these years.
