What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued

What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Projector Headlights — Toyota Tundra (2014–2021)
A single bi-LED projector in each lamp handles low and high beam, with a sharp cutoff, wide spread and high intensity. Around it, LED lines on the top and bottom edges form the running light, and the housing has a blacked-out look behind a UV-coated polycarbonate lens.
The turn signal chases in amber across the top and bottom, then returns to the white DRL. Dedicated connectors plug into the factory harness for each function, a separate connector keeps the factory auto-leveling working, and a fuse-box DRL harness runs the DRL whenever the truck is on. If you would rather keep a reflector look, Form also makes a reflector version for these years.

No. A plug-and-play connector is included to retain the factory auto-leveling feature.
Are these street legal?
Form Lighting states they meet SAE, DOT, FMVSS and CMVSS standards for on-road use in the US and Canada, as long as they are installed and aimed properly. Integrated screw-type adjusters handle the aiming.
