Form Lighting Warranty
At a glance: 2-year limited warranty on every Form Lighting product against manufacturing defects and functional failure, plus free lifetime tech support
This is Form Lighting's policy, not an NVMOS policy. We've summarized Form Lighting's published warranty in plain English. The manufacturer's official terms always govern and can change without notice. Last reviewed on nvmos.com: October 15, 2026.
Coverage by product
| Product or component | Coverage |
|---|---|
| All Form Lighting headlights, tail lights, fog lights and accessories: manufacturing defects or functional failure | 2 years from purchase |
| Product support and troubleshooting, including after the warranty runs out | Free, for life |
| Moisture beyond normal condensation (running droplets or puddling) | Covered at Form's determination |
| Consumable parts such as incandescent bulbs | Not covered |
What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Tail Lights — Toyota Tacoma (2024–2026)
A tri-bar LED tail light for the fourth-gen Tacoma, styled after the lamps on the third-gen Tundra. All three bars light for running, brake and turn, and they sweep outward in sequence when you signal.
Pick a red lens or a smoked one. Both use red LEDs and stay road legal. Form Lighting rates them plug-and-play on every 2024–2026 trim, with no body gaps, no dash errors and no hyperflash, so the job is mostly unbolting the old lamps.
| Spec | Detail |
|---|---|
| Optics | Tri-bar layout with automotive-grade LED optics |
| Functions | Running, brake, sequential turn, retroreflector sidemarker |
| DRL | Running light function with integrated sidemarker; no separate DRL |
| Turn signal | Sequential (all three bars sweep outward), switchable to static |
| Install | Plug-and-play on all trims, about 10–20 minutes, basic tools |
| Compliance | SAE and DOT compliant for on-road use |
| Assembly | UV-coated polycarbonate lens, automotive-grade LEDs, IP67 weatherproof |
| Warranty | 2 years, US-based support |
| Fitment | Toyota Tacoma 2024–2026, all trims |
| Part number(s) | FL0108 (Red), FL0109 (Smoked) |
Does this fit every 2024–2026 Tacoma trim?
Yes. Form Lighting lists it as plug-and-play for all trims of the 2024–2026 Tacoma.
Can I turn off the sequential turn or startup animation?
Both can be disabled during installation, leaving a static turn signal and no startup sequence.
Smoked or red?
Both use red LEDs and are SAE and DOT compliant. Smoked reads darker against dark paint; red is closer to the factory look.
Will these trigger dash warnings?
Form Lighting states there are no system or dashboard errors, no flicker and no hyperflashing.
