What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued

- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Reflector Headlights — Toyota Sequoia (2008–2017)
The 2008–2017 Sequoia came with halogen headlights and no running light. This pair replaces them with full LED low and high beams, an integrated white DRL, and an amber turn signal that sweeps across the middle of the lamp before handing back to the DRL. The styling borrows from the 2018-and-later Sequoia.
The main functions plug into the factory headlight connector, and an included fuse tap powers the DRL. A small trim bezel fits under each lamp and can stay matte black or be painted. Assembled in an IATF 16949-certified facility.
