What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued

- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Projector Headlights — Ram 1500 Classic (2019–2024)
For the Ram 1500 Classic, Form uses the same lamp it builds for the 2009–2018 trucks. Two dual-beam projectors per side handle low and high beam with a sharp cutoff and broad width, behind UV-coated polycarbonate lenses in an IP67-rated housing.
At startup the LEDs sweep across the top and bottom before the running light fully comes on, and the turn signal sequences in amber from the inside out. It fits Classics with factory halogen or LED lamps, using the included adapters on halogen trucks, plus a fuse-box DRL harness for trucks without factory DRL. Big Horn trims are not compatible.

Yes. Form sells the same part, FL0016, for both, which is why the part name references 2009–2018.
Will this fit the new-style 2019+ Ram 1500?
No. This is only for the Ram 1500 Classic. Form makes a separate lamp, FL0015, for the 2019–2024 Ram 1500.
Are these street legal?
Form Lighting states they meet SAE, DOT, FMVSS and CMVSS standards for on-road use in the US and Canada, as long as they are installed and aimed properly. Integrated screw-type adjusters handle the aiming.
