Form Lighting Warranty
At a glance: 2-year limited warranty on every Form Lighting product against manufacturing defects and functional failure, plus free lifetime tech support
This is Form Lighting's policy, not an NVMOS policy. We've summarized Form Lighting's published warranty in plain English. The manufacturer's official terms always govern and can change without notice. Last reviewed on nvmos.com: October 15, 2026.
Coverage by product
| Product or component | Coverage |
|---|---|
| All Form Lighting headlights, tail lights, fog lights and accessories: manufacturing defects or functional failure | 2 years from purchase |
| Product support and troubleshooting, including after the warranty runs out | Free, for life |
| Moisture beyond normal condensation (running droplets or puddling) | Covered at Form's determination |
| Consumable parts such as incandescent bulbs | Not covered |
What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Projector Headlights — Ram 1500 (2019–2024)
Three bi-LED projectors per side cover low and high beam on the fifth-generation Ram 1500, with blacked-out housings, UV-coated polycarbonate lenses and an IP67 rating. A two-stage chasing sequence plays each time the running lights come on.
Amber LEDs in the lower section light from the inside out when you signal. They are designed to fit trucks with either halogen or factory LED headlights, with adapters that plug into the factory connector. Two fitment notes worth reading before you order: they do not fit Big Horn, Longhorn, Limited or TRX trims, and trucks with the Premium Lighting Group need the DRL turned off in the media screen settings for the low beams to work properly.
| Spec | Detail |
|---|---|
| Optics | Three bi-LED projectors per headlight (low and high beam) |
| Functions | Low beam, high beam, DRL, sequential turn signal, startup sequence |

| Turn signal | Sequential amber in the lower section, inside to outside |
| Install | Plugs into the factory connector via adapters; 1–2 hours, basic tools, basic wiring only |
| Compliance | Meets SAE, DOT, FMVSS and CMVSS standards for on-road use in the US and Canada when installed and aimed properly |
