What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Projector Headlights (Amber DRL) — GMC Sierra 1500 (2014–2018)
GM put some LED into the 2014–2018 Sierra 1500 headlights but stopped short of full LED. This pair finishes the job with a triple-projector LED low and high beam, blacked-out housings, and smoked lenses over an amber running light, turn signal and sidemarker.
They bolt to the factory mounts and plug into the factory connectors, and an included harness adds the DRL to trucks that never had one. Assembled in an IATF 16949-certified facility. Trucks with factory LED turn signals are not compatible.

Will these fit my Sierra?
They fit the 2014–2018 GMC Sierra 1500. They are not compatible with trucks that came with LED turn signals from the factory. The 2015–2019 Sierra 2500/3500 HD has its own listing.
Amber DRL or white DRL version?
Fitment is the same; the difference is styling. This version runs an amber DRL behind smoked lenses with an amber startup. The standard projector version uses a bright white C-shaped DRL and a white startup chase.
What comes in the box?
Left and right headlamps, a DRL harness that plugs into the fuse box, and two halogen headlamp adapters. Form quotes one to two hours with basic tools and no modification.
Are they street legal?
Form states they meet SAE, DOT, FMVSS and CMVSS standards for road use in the US and Canada, provided they are installed and aimed with the built-in screw adjusters.
