What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued

What's not covered
- Normal condensation on the lens, which clears as the lamp warms up

- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting Sequential LED Projector Headlights — Ford Maverick (2022–2024)
The factory Maverick headlamp is a reflector. This one swaps in three projectors per side: two bi-LED units for low and high beam and a third that adds reach with the high beams on, for a sharper cutoff and more usable light. The housing is fully blacked out with the factory chrome gone, behind a UV-coated polycarbonate lens.
A startup sequence runs when the C-shaped running light comes on, and the turn signal sweeps sequentially. Both can be switched off during install if you prefer a plain DRL and a static blinker. Adapters cover Mavericks without factory daytime running lights, and the lamps plug into the factory connector.

Can I turn off the sequential turn signal and startup sequence?
Yes. Both can be disabled during installation, giving you a static turn signal and a DRL that simply switches on.
My Maverick has no factory DRL. Will the running lights work?
Yes. Form includes adapters that power the running lights on non-DTRL trucks.
Are these street legal?
Form Lighting states they meet SAE, DOT, FMVSS and CMVSS standards for on-road use in the US and Canada, as long as they are installed and aimed properly. Integrated screw-type adjusters handle the aiming.
