What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued

- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Projector Headlights — Chevrolet Silverado (2007–2013)
The 2007–2013 Silverado shipped with halogen headlights, and they are the weakest part of the front end. This pair replaces them with three bi-LED projectors per side, a C-shaped white running light the factory lamps never had, and an amber turn signal that sweeps from the inside out.
They plug into the factory headlight connector and bolt in without gaps. Form builds them in an IATF 16949-certified facility and states they meet SAE, DOT, FMVSS and CMVSS standards once installed and aimed. They are for US-market trucks only and do not fit the Silverado Classic.

Will these fit my Silverado?
They are built for the 2007–2013 Chevrolet Silverado sold in the US. Form excludes the Classic body style, and its listing does not break out 1500 versus 2500HD/3500HD trims, so check with us before ordering for an HD truck.
Projector or reflector version?
Both replace the halogen lamps with full LED, a white C-shaped DRL and a sequential turn signal. This projector version uses three bi-LED projectors per side for a sharper cutoff. The reflector version uses LED reflector optics if you prefer that look.
Do I need the DRL harness?
Only if you want the running light. The factory headlights have no DRL, so the included harness plugs into the fuse box to power it. The main beams and turn signal work off the factory connector.
Are they street legal?
Form states they meet SAE, DOT, FMVSS and CMVSS standards for road use in the US and Canada, provided they are installed and aimed correctly with the built-in adjusters.
