Form Lighting Warranty
At a glance: 2-year limited warranty on every Form Lighting product against manufacturing defects and functional failure, plus free lifetime tech support
This is Form Lighting's policy, not an NVMOS policy. We've summarized Form Lighting's published warranty in plain English. The manufacturer's official terms always govern and can change without notice. Last reviewed on nvmos.com: October 15, 2026.
Coverage by product
| Product or component | Coverage |
|---|---|
| All Form Lighting headlights, tail lights, fog lights and accessories: manufacturing defects or functional failure | 2 years from purchase |
| Product support and troubleshooting, including after the warranty runs out | Free, for life |
| Moisture beyond normal condensation (running droplets or puddling) | Covered at Form's determination |
| Consumable parts such as incandescent bulbs | Not covered |
What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Puddle Lights are a plug-and-play pair that replaces the factory puddle lights on 2019–2026 GMC Sierra 1500 and Chevrolet Silverado 1500, plus 2020–2026 Sierra and Silverado 2500/3500. They light the ground beside the doors, so you can see where you are stepping at night.
Why it works
Each light puts out 65 measured lumens on 1.4 watts through a clear, UV-coated polycarbonate lens. They use OEM-size connectors, so installation is a swap rather than a wiring job, and they are IP67 rated for life on the outside of the truck.
