What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

Form Lighting LED Dually Sidemarkers are a plug-and-play set that replaces the factory dually fender sidemarker lights on 2010–2018 Ram 2500/3500 dually trucks. They use LED amber and red output to mark the width of the truck at night.
Why it works
A dually is wide, and the fender markers are what tell other drivers how wide. These run 20 lumens amber and 25 lumens red while drawing only 1.2 watts (0.1 A). They use OEM-size connectors, so it is a straight swap, and they are IP67 rated for road spray and washes.
