Form Lighting Warranty
At a glance: 2-year limited warranty on every Form Lighting product against manufacturing defects and functional failure, plus free lifetime tech support
This is Form Lighting's policy, not an NVMOS policy. We've summarized Form Lighting's published warranty in plain English. The manufacturer's official terms always govern and can change without notice. Last reviewed on nvmos.com: October 15, 2026.
Coverage by product
| Product or component | Coverage |
|---|---|
| All Form Lighting headlights, tail lights, fog lights and accessories: manufacturing defects or functional failure | 2 years from purchase |
| Product support and troubleshooting, including after the warranty runs out | Free, for life |
| Moisture beyond normal condensation (running droplets or puddling) | Covered at Form's determination |
| Consumable parts such as incandescent bulbs | Not covered |
What's covered
- Manufacturing defects and functional failures for 2 years: Form replaces the product, or a comparable one if it has been discontinued
- Single-component problems: Form may replace just the part needed to restore function
- Coating damage that shows up within the warranty and traces to a manufacturing defect
- Form pays to ship the replacement to US customers, and usually doesn't need the old part back
What's not covered
- Normal condensation on the lens, which clears as the lamp warms up
- Surface finishes worn out by normal use
- Installation labor or time spent swapping a warranty part
- Improper installation or use, abuse beyond normal wear, or any opening, rewiring or modification of the lamp
- Purchases from unauthorized sellers. The warranty is non-transferable
How to make a Form Lighting claim
- Contact Form Lighting before you take the lamp to a shop; they'll diagnose the problem with you first
- If Form confirms a defect, they issue an RMA number. Nothing goes back without one
- For moisture claims, include photos that show how much water is inside the lamp
- If Form does ask for a return, you pay the return shipping

The Form Lighting LED Dome Light is a plug-and-play replacement for the factory overhead lamp in the 2009–2023 Ram 1500, and it also fits the Ram 2500/3500 from the same years. It puts LED output behind a clear lens and installs without any wiring changes.
Why it works
Interior lighting is one of those things you do not think about until you are digging for a dropped socket at night. This unit puts out 195 measured lumens while drawing just 1.9 watts (0.15 A), a low draw that is easy on the battery if a door gets left open. It uses an OEM-size plug, an FR4 circuit board and a UV-coated polycarbonate lens, and it is rated IP67.

Connects with an OEM-size plug-and-play connector. If your truck has an unusual overhead console, send us a photo before ordering.
